Bulgarian to English translations [PRO]

Bulgarian term or phrase: Много
This term is from a back translation of a medical questionnaire on symptoms of Crohn's disease. Here are some of the pertinent questions:

- През изминалите седем дни по-често ли имахте газове от обикновено?
- През изминалите седем дни имахте ли болки в корема?
- През изминалите седем дни чувствахте ли се уморен(-а) и без енергия?

And here is the range of responses the patient has to choose from:

- Изобщо не
- Малко
- Донякъде
- Доста
- Много

Beyond the proper translation of "малко" in that range of responses, I'm also wondering whether those responses are appropriate for Bulgarians.

Explanation:
Since this is the top of the scale, in my opinion this should be "extremely". We don't have a proper form for this, except perhaps "ужасно много" but we won't put that in an official publication, like a questionnaire. Although "извънредно" seems to be more appropriate, it will sound awkward in my ears.

"Изключително много" would also have been appropriate, but one, it's too long, two, it adds nothing to the meaning. I guess Bulgarian is more "reserved".
