There are few mistakes int he source text but I think I managed to decipher the meaning.

the last think is the name of the town in CZ: Olomouc 7 (i.e. specific address).

the initial verb depends on the subject matter. I guess it was a sculpure, a piece of art. And now it depends what was meant by the Czech verb "provedly". This actually means the company Kosmos did it, manufactured it, executed... depending on the material, shape and style of the piece (if it really is a piece of art, which i think it is).
