Explanation:
Both are house numbers. One is - or should be a serial number of the house within the district/community, the other is a serial number of the house within the street. Not sure but don't think there are any approved/certified translations for this - up to us to coin our own.

Vycházím z české definice čísla popisného, které se vztahuje pouze na číslo budovy (užívá se i u budov, které se nalézají mimo pojmenovaných ulic, např. dům mých rodičů čp. 200) a čísla orientačního, které se vztahují k ulicím a popř. k náměstím, např. druhé číslo za lomítkem: Štefánikova 2111/15. Terminologii v anglickém jazyce bych tedy moc nekomplikovala, i přesto že se někdy užívají oba termíny jako synonyma.
