Danish term or phrase: s. adr. deptm. med.
This list of abbreviations is in a table labelled "Udenbys flytninger" (Out-of-Town Moves) and then under a column labelled "Bopæl" or "Address." It appears a few times, and always when out-of-town means a completely different country. It appears once as "deptm. med." and twice as "deptm. medd."

I'm uncertain of the type of document, as there is not title to it, nor any sort of organization name. It does include columns for membership dues and health insurance information.

Any help that could be provided on these abbreviations would be most appreciated. I've exhausted al my resources!

Explanation:
sidste adresse departement meddelt

These are not standard abbreviations, but some particular person or organisation or whatever's choice.

My suggestion is just a guess, and without context, it is difficult to know what this should mean. I cannot think of any other word than 'departement' that could be abbreviated as 'deptm'.
