I translated this as "For history: Treatment contact date 05 Oct 2016."
Evidently 05 Oct 2016 is indeed the date of the discharge.

I don't agree. I think it means "For the patient's chart" (ENUS). But you get 3 points for effort and for possibly being right.
Notes to answerer
Asker: The phrase is at the total end of the report. There is nothing after it. Are you saying that the phrase describes the person who signed the report? (In one instance the document is not a discharge summary but an evaluation of a patient by a physician's assistant. I'm not even sure the PA actually examined the patient; she may have been summarizing the chart.)
