It is from a financial report for a certain country...

Here is the context:

The latest Debt Sustainability Analysis prepared for X country in (x year) concluded that, without the delivery of Y debt relief fund, the country will remain in debt distress. The baseline framework, which assumes delivery of this grant, indicates a significant and ***sustained breach of the relevant thresholds.***

I know what it means but don't know how to put it in Albanian so that it conveys the idea exactly as in English.

What it means is this:
A baseline framework was set up for the government to follow in order for the country to get the grant. Certain things had to be done and done right, but the report says that " a sustained breach of the relevant thresholds" happened = important threshholds/steps of this process were not followed as they should have been ....which of course has caused serious problems for the country in getting this grant.
