In modern times, the use of فشل for "failure" has become so common as to be practically acceptable. I prefer إخفاق for the sake of semantic precision.

It is tempting to plug in a one-word modifier, parallel to "initial," but my ears just couldn't tolerate it, so I took the liberty to use a more Arabic-sounding expression. It is a matter of style, I suppose.
