Summary: On 16 June 2019, Mrs. xxx came to BI Hospital and had oncology consultation. She has been diagnosed with CLL from 2017, on holding Brigatinib from pre-operation (May 2019). She had EGD in another local hospital in Bangkok on 23 March 2019 where pathology report rendered atypical cell suspected malignancy: Linitis plastica?. She underwent distal gastrectomy + Roux-en-Y, cholecystectomy and intraoperative cholangiography on 30 May 2020. Pathology reveled adenocarcinoma diffuse/Signet-ring cell CA grade3 invade to serosa, HER2: negative, margin: (distal positive involve cancer), and LVI/PNI: positive. On consultation, she complained of bilateral leg swelling. Impressions were CLL and gastric carcinoma stage4. Further investigations were ordered. CBC showed Hb 7.1 g/dL and Hct 21%. Consequently, she was admitted for blood transfusion.
