English term or phrase: Carer's Asessment
Carer’s Assessment
You are a Carer if you regularly provide or intend to provide a substantial amount of care for a friend or relative. Whilst XXX has the responsibility of offering you a Carer’s Assessment, it is your legal entitlement to request that an Assessment of your needs be carried out. If the person you care for is assessed as needing services from XXX then you are also eligible to have a Carer’s Assessment of your needs, even if that person does not avail of their services.

What is a Carer’s Assessment?
A Carer’s Assessment is a shared understanding between you and your key worker as to your caring role or responsibilities. It will take account of your circumstances, needs and views. The Assessment is not a test of your financial situation, it is a conversation for you to focus on how caring impacts on you and your lifestyle and the information discussed can be compiled over a number of visits.

carer's asessment

Explanation:

1 day 21 hrs

carer's asessment

Explanation:

Notes to answerer

44 mins
