Daarbij wordt er geprobeerd via een zo klein mogelijke incisie te werken. Als het proximale deel van het femur goed zichtbaar is, wordt een bothevel geplaatst achter de trochanter major. De mergholte wordt geopend en met een femurrasp wordt de mergholte van het femur vergroot en verwijd zodat de ruimte groot genoeg is voor de prothese die geplaatst gaat worden. Femoral canal preparation The Corail broach is available with four broach handle options depending on the surgical approach (Figures 4A, 4B, 4C); dual-offset handle also available, but not shown. Select the appropriate handle for the surgical approach. Beginning with the smallest Corail compaction broach attached to the selected broach handle, progressively enlarge the metaphyseal cavity by compacting and shaping the cancellous bone until the level of the neck resection is reached. Broaching should continue until complete stability is achieved with the last size broach used without reaching cortical contact in the femoral canal, ensuring cancellous bone preservation. The double-offset handle is used to facilitate the broaching of the femoral canal. The direct anterior approach requires the availability of specially curved, angled, or offset instruments. Always use the smallest broach available, and always use a double-offset broach handle to broach the femoral canal. Using minimal force, insert the broach handle by hand into the open canal. When the broach is completely aligned with the femur, tap it with the hammer. The double-offset design of the broach handle eases the introduction of the broach because it reduces the need to elevate the proximal femur. This broach design also greatly facilitates surgery for obese and muscular patients when leverage of the proximal femur to skin level cannot be achieved. There are two versions of this broach handle, for the left and the right leg. This lateral offset reduces the need for adduction and elevation of the operated leg. It allows broaching the canal with the proximal femur remaining a few centimeters below skin level ( Fig. 5 ). The first broach can be used gently to lateralize further the subsequent broaches.
