Από παθοφυσιολογική άποψη, ο όρος «διήθημα» παραπέμπει σε «παθολογική ουσία που συσσωρεύεται σταδιακά στο εσωτερικό κυττάρων ή ιστών του σώματος» ή σε «κάθε ουσία ή τύπο κυττάρου που εμφανίζεται στο εσωτερικό ή εξαπλώνεται μέσω των διακένων (διάμεσος χώρος ή/και κυψελίδες) του πνεύμονα, που είναι ξένη/ο για τον πνεύμονα, ή που συσσωρεύεται σε μεγαλύτερη της φυσιολογικής ποσότητα στο εσωτερικό του».
