All titles/honorifics like Dr., His Excellency, Honourable, Professor, etc., are conferred titles. Same is true of Mr., Mrs., Ms., etc., they are conferred by society on people confirming to certain norms of society - "not married", "married", "male", "female" etc, or for acquiring certain distinctions, educational qualifications, etc.

When you say that, the context changes. Certain word choices would be appropriate for a form or a website (उपाधि, for example), a different word choice (संबोधन) would be appropriate in a conversation or a a face to face talk.

If you want to use उपाधि in a conversational setting, you would have to say something like:

आपके नाम के साथ क्या उपाधि लगाना ठीक रहेगा?

The answer to your other question: आपको कैसे संबोधित करूँ would not necessarily be श्री, श्रीमान, कु, इत्यादि. It could also be, आप मुझे महाराज, गुरु, भाई साहब, आंटी, सेवक, प्रधान मंत्री, भगवान, आचार्य, अध्यक्ष, देवर, राखी भाई इत्यादि कहकर संबोधित कर सकते हैं।

I don't see any problem if a drop down list containing श्री, श्रीमती, कुमारी, डॉक्टर, etc is named उपाधि.

Asker: isnt उपाधि something that is conferred to a person after it has been 'earned'? the context is a webiste where information is to be inserted

Asker: just for my understanding, during a conversation if i was to say: how do i address you, would you use upaadhi? मैं आपको कैसे संबोधित करूं? अथवा मैं आपको क्या उपाधि दूं?

Asker: so in a drop-down selection box for a website one should put उपाधि any examples where this may happen? how is it mentioned on hindi application forms? i seem to remember that all 3-4 types of title shree/shrimati etc. were just separated by means of slashes
