Many times family and friends do not really believe the person with narcolepsy has not control over being sleepy or falling asleep. They often think that if the person tried a little harder, really cared or showed a little more interest, he or she could stay awake. They think the person should be capable of doing all the things as B.N. (Before Narcolepsy). If others knew the facts about narcolepsy, they would know this is not so (è tutto al passato?”se aveesero conosciuto…avrebbero saputo…”?). when others make unreasonable demands upon the person with narcolepsy, they set in motion another chain reaction. Lack of understanding or the perception thereof is a two-way street. Not only does the person with narcolepsy feel misunderstood by others but that feeling is reciprocated. Others also feel they are not understood. (è possible che B.N. significhi “la persona non narcolettica, prima di ammalarsi”??e tutti gli "others" sono riferiti a "family and friends" o alle persone che non offrono del disturbo della narcolessia??lo traduco semplicemente con "altri"??)

Per quanto riguarda il "knew": è l'equivalente del congiuntivo italiano, se conoscessero le manifestazioni della narcolessia, allora saprebbero che.....

Others:
certo che devi tradurre con "gli altri", cioè tutte le persone "altre, diverse" dal paziente.

Sul B.N. non so risponderti purtroppo (la tua interpretazione è comunque possibile).
