Non metterei "irreparabili" come suggerito, dato che non sappiamo dal punto di vista medico se lo sono o meno. Suggerirei invece di rimanere fedeli all'inglese ed utilizzare "disastrosi". L'interpretazione generale è quella di fare danni enormi, disastrosi (alle condizioni di salute del paziente).

Il contrasto in questa frase sta nel fatto che certe funzioni (cognitive?) siano state preservate nonostante la malattia abbia già causato danni irreparabili/irreversibili nel paziente.

Il verbo 'to wreak havoc' è molto forte e ha una certa connotazione di violenza, come la 'forza distruttiva' di un uragano che devasta senza guardare in faccia a nulla e nessuno. Il fatto che venga utilizzato in un testo di psicologia ci dovrebbe far pensare a quanto disastrosa possa essere la situazione di questo paziente, per definirla in questo modo...

Sicuramente ce ne sono molte altre, ma adesso non mi viene niente di meglio, sorry! :(

'disastrosi' mi suona male in italiano. Anche 'disastrous havoc' non è ottimo inglese. 'Disastrous' è ridondante', meglio solo 'havoc'.
