Ja bym to jednak rozumiała tak, że w formularzu pytają kandydata o to, w jaki stopniu chce opanować język, więc gdybym miala wyrazić tę myśl, to napisałabym tak:
Jaki poziom znajomości języka chciałby/chciałaby Pan/Pani osiągnąć? lub
W jakim stopniu/ na jakim poziomie chce Pan/Pani opanować
język?
Nie wiem, co jest jeszcze w tym formularzu i nie potrafię się odnieść do "requested languages".
Języki, których chce się Pani/Pani uczyć? (to może być błędny trop, bo może chodzić o to, że zglaszający się na kurs może sobie wybrać grupę, w której będą przeważać ludzie posługujący się wskazanymi językami). Nie wiem.

Może w ogóle to sparafrazować i zostawić tylko: wybrany język i poziom zaawansowania (pewnie grupy, do której ktoś chciałby się przyłączyć), co już kolega wyżej zasugerował (poziom zaawansowania).
