Me parece que la opción "Cobro por cesantía del empleo" es muy correcta. Es mejor no especificar y ceñirse al original. Además, como bien dices, si se trata de un formulario hay que ser breve.

Asker: Marta, muy agradecido por su ayuda. Necesito elegir un término algo sucinto ya que se trata de una opción en un formulario. ¿Que le parece "Cobro por cesantía del empleo"? No creo que haya que especificar que es por servicio militar, tal vez temporal, ¿no?
