I'm translating a series of text messages to be used in a text-based home COVID monitoring program in the US (probably mostly the Philadelphia area), so I need to keep things as brief as possible. The message to be translated is "Before we unenroll you, please help us improve this program by answering a few questions.

Here's what I've got:

Antes de darte de baja [I'm using informal for all these messages, at the client's request], ayúdanos a mejorar este programa contestando unas preguntas.

My question is whether "darte de baja" is appropriate for this context, or if I would be better using "cancelar tu inscripción" (or perhaps suscripción? Patients have to sign up for/opt into the program.) Obviously, the former would be shorter, but I wasn't sure if it's only used for school enrollments.
