A term "caregiver" in a birth certificate?

As for it being too long for a birth certificate - there are no caregivers in Ukraine, as far as I know, unless among hospital staff, probably, but that's different from what the term means in the US context - so how can there be an exact term in Ukrainian?

Do they mean something closer to "legal guardian"?

Notes to answerer
Asker: It is essentially what this person does. The extended version is too long for the birth certificate but thank you very much for your suggestions.
