میڈیکل کے لئے طبی موزوں لفظ ہے۔ ہسٹری کے لئے تاریخ، سرگزشت یا ہسٹری مناسب لگ رہے ہیں۔ لیکن ان تینوں میں میرے خیال سے سرگزشت سب سے بہتر ترجمہ ہوگا۔

گذشتہ طبی روداد/سابقہ طبی احوال/طبی تاریخ


Explanation:
I do not agree with سرگذشت
instead, I recommend the word "گذشتہ طبی روداد"
یا
"سابقہ طبی احوال"

otherwise the word "طبی تاریخ" is much better than "طبی سرگذشت"

agree  MOHD AYYUB: "گزشتہ طبی روداد" conveys the meaning and easy to understand for common people. "سرگزشت" is also common for "History" but "گزشتہ روداد" is easier for masses. It is my assumption.

Explanation:
The appropriate Urdu translation is 'طبی سرگزشت' but some people might not be familiar with it. Transliteration is also popular to a majority of Urdu speaking community.

So, the best way is to use the translation while keeping the transliteration within brackets, at least for first time in the whole document. For example:

Explanation:
I personally prefer transliterating this term, since "medical history" is now a commonly used and widely understood term in Urdu. However, if you have to translate this, I feel 'طبی کوائف' covers this term more accurately, rather than a literal translation of 'medical history' as 'طبی تاریخ'.

At the end of the day, I guess it depends on the context too.

  -> Don't want to nitpick, but doesn't 'particulars' mean 'details'? What is medical history, if not details about someone's health?

agree  Qudsia Lone: Agreed. طبی کوائف is a generally used term common to Urdu which includes medical history. To make it more specific to "history" perhaps: سابقہ طبی کوائف
