neutral  B D Finch: Not all hatches are horizontal, e.g. serving hatches between kitchens and dining-rooms.

  -> Certainly, but I wasn't giving a dictionary definition. I would expect a fortress hatch to be as in my explanation.

agree  Yvonne Gallagher: It could be a vertical hatch or door as BDF says but I think this is more likely in the context. Often a hatch used either for dungeon/cellar or in tower/keep between floors
