Child's Name: Rhiannon xx. County: Polk. Date of Birth: ...

Original text:
The family's involvement with the Department began in 2004, the report was closed with no indicators of failure to protect, sexual battery, substance exposed child and environmental hazards as to the child Rhiannon. In 2006, the mother was a significant other on case that was closed verified for family violence. In 2011, the mother admitted to being addicted to methamphetamines and had been using since she was 21 years old. The case was closed not substantiated for substance misuse with the mother being referred to Tri-County for substance abuse services. In 2012 and 2013, there were two child on child reports related to the child, Rhiannon. In 2015, the Department closed a report not substantiated for substance misuse. At the time there was a step father in the home who had a substance abuse problem but the mother was clean from drugs at the time. The most recent investigation related to this family was in 2016 which was closed no indicators of inadequate supervision, physical injury but verified for sexual abuse. The mother had gone to buy groceries and left the child home alone for about one hour, the child, Rhiannon, invited neighborhood adult males into the home and had sex with them.

I believe what is meant is: "In 2006, the mother was a significant other on case that was closed (having been) verified for family violence."

Other places they talk about "closed substantiated for substance misuse" "closed no indicators of inadequate supervision ...".
