Hi I don't really understand this joke. Could anyone help me to get it? Thanks!

Context:
A cured patient is a lost source of income.
A sick patient who is marginally "improved" is a manageable patient.
The joke:
This guy’s doctor told him he had six months to live.
The guy said he couldn’t pay his bill.
The doctor gave him another six months.”

The doctor warned the patient that he only had 6 months left to live (because of his illness)

When the patient said the he couldn't afford to pay his bill, the doctor then said "Oh, well, you've got 12 months to live" — meaning he would have more time to find the money to pay his bill; possibly, also, more time to build up even more medical expenses.

The point being of course the doctors' prognoses on life expectancy are notoriously inaccurate, so the fact the doctor changed his mind also reflects the fact that he probably didn't really know anyway.

I think this joke is probably US, because of their healthcare system and the concerns over being able to pay medical bills etc.
