In the light of your note, I think it is likely to be along the lines of a control number. Since it is given merely as an abbreviation, seeems to indicate that it is of no special significance to the patient.

Another possibility which comes to mind is "screening control number" (idea came from this link: http://216.239.59.104/search?q=cache:oO9I3XrxW3IJ:sitemaker.... )

Just to be sure (I had never heard of this before I searched about your question!!), it can't be Suprachiasmatic Nucleus, can it? I gather that the patient would have even less knowledge of this!

Asker: It's a form for registering menstrual flow, where the patients writes down the presence of bleeding, clots/flooding, number of pads/tampons used, presence of discharge and associated pain. This is presented as a generic sample of this kind of form in a text about abnormal uterine bleeding.

Suprachiasmatic nuclei


Explanation:
SCN is that part of the brain that functions as a person's "biological clock" to regulate many body rhythms. The SCN is located on top of the main junction of nerve fibers that connects to the eyes.
