I think 'association' might be an appropriate translation?

(teachers') mutual (insurance company)
Explanation:

The acronyms listed in your question are all either teachers' mutual insurance companies (MGEN) or teachers' unions or orgs linked to teachers, health benefits, etc.

Anyone interested either in insurance or in Africa, or both, as I am, will tell you that the creation and the development of mutual ins cos in Africa (secteur mutualiste) have been on the agenda for... quite a long time. Bit of an old story, that one. Projet en cours says it all.

Regarding the context of your question, depending on what your text says, you could add "health" to "insurance company"

"So would a "femme mutualiste" be a woman benefitting from mutual insurance?"
This phrase appears to be used in French-speaking Africa - you may be right and it could also describe women who are involved in developing mutual ins cos, le mouvement mutualiste, etc. It all depends on how it is used.
(I meant to add this to the discussion but I can't.)

-> Thks. What if there's no "primary" insurance - e.g. if there’s no public/state ins system as we know it? Also, "secondary" does not necessarily imply that it is mutual (can be private),as stated in source text.
