The above term is the title of a form that I need to translate concerning health and safety and what to do in the event of an accident. I'm just not sure how 'reflexe' fits into this. It seems a bit superfluous to me as 'Action Form' sounds alright. Or perhaps 'Action Report Form', however, neither of these options captures the sense of 'reflexe'.

Would anyone have any ideas/suggestions?
