This is a femoral approach used during total hip revision to access loosened femoral components and remove cement prior to placement of a new prosthesis. Here's nice big chunk of context for you to get your teeth into!

"Préparation de l'intervention: établir une stratégie opératoire.
Etablir une stratégie, c'est choisir, dans un premier temps, une voie d'abord fémorale qui permet de surmonter les différents obstacles dépistés lors de l'analyse radiologique tout en respectant les objectifs imposés par le concept press-fit. De ce choix dépend la zone du fémur où s'effectuera la stabillité primaire de l'implant.

"endo-femorale" is used everywhere after that.
But I am not at all happy with "endofemoral" in english - So far I have "transfemoral" - but am not 100% sure - is anyone out there familiar with these surgical approaches?

Not a lot of Google hits either but it seems to me that "transfemoral" is the same as "transcortical". Sorry I don't have time right now to delve deeper.
