Context is a document detailing all the costs/charges involved in a clinical research trial. The term comes up many times, as does "refléchage". Some context:
"fléchage CHU [Centre hospitalier universitaire de Nantes]"
"c'est déjà mentionné dans la partie Pharmacie, lignes 208 & 209, avec possibilité de flécher vers le service qui contacte réellement l'IVRS"
"sinon, refléchage interne"

I get the impression it is talking about who is responsible for certain specific costs (the host hospital vs the study's corporate sponsor, etc.), but I haven't been able to find any examples of its use in this kind of context.

Le "fléchage" est un terme de jargon interne aux organisations de recherche clinique, qui reste encore difficile à trouver sur Internet, mais qui est couramment utilisé dans le contexte des "grilles de surcoûts", c'est-à-dire des budgets des contrats d'essais cliniques au moment de la négociation avec les centres d'étude. Le fléchage consiste à affecter telle ou telle somme à tel ou tel poste budgétaire ("earmarking" est donc très bien en ce sens).

Par extension, on l'utilise aussi pour exprimer le fait-même d'intégrer un coût à une grille (budget). Comme les modèles de budgets standard ne prévoient pas toujours tous les coûts, il faut les "flécher", c'est-à-dire définir où et à quel niveau les intégrer. L'enjeu est important : si un coût n'est pas intégré, certains travaux ou certains intervenants dans l'essai risquent de ne pas être rémunérés.

Thank you! I think this is the most generic and correct translation in this context. Earmarking is also appropriate in many cases as you mention, but doesn't address "re-fléchage". Thank you for the thorough referencing as well!
