here's a fun one for you. There's a slightly sexual double-entendre here, 'Blasen'=bladder and blowing 'Tuten'= aural canals and tooting. Coupled with the idea that neither have a clue. Thoughts?

I wonder whether the idiom might be mostly a play on words, with an intentional double-endendre being the core of the joke. It might not really be about incompetence at all, but rather a juvenile joke about certain professions that might be funny to a specific crowd. Somebody came up with the idea that 'Tuten und Blasen' sounds funny in connection with the corresponding medical specialties and also has a sexual undertone. I am not sure how to translate a wordplay, put I think any joke about about professions with a sexual undertone will do.
