for all your input. In the end I went with @Lancashireman's suggestion and omitted it in the question, as it worked best in the context. I did go for @Johanna Timm's suggestion of the literal translation "passport" and have awarded her the points. Pity I didn't see @Mair A-W's suggestion until after the job was delivered because I translated "bonus heft" with dental health record book".

If it's Germany, they'll be hard copies, with at least one stamp ... :-)

Also, I have one of them, the Bonusheft, and it can certainly be described as a card. It's rather like those café loyalty cards: get it stamped ten times and you get a bonus. (... not *exactly* so, but...).

...why people are trying to avoid the word "pass(port)" once, so they can repeat it four times in the form of list items.

[Forget about the middle part; didn't see the "new" at first. Nevertheless, some of these "new patient questionnaires" look really weird. The "medical history form" looks much more like the German version: https://www.nhsdentalpractice.co.uk/pdf/medical-history-form...

You might as well say the following passports (or whatever) and use "Other (XY)" for the Bonusheft (if you need this at all).

@ MM. Thanks for the thumbs-up. "Omit" is not a popular option among askers who generally feel obliged to render their clients' every word into EN. Also, you're so far ahead as to be uncatchable.

Do you have any of the following?

The list contains items that might be described as cards/passes/passports, but the first one (Bonusheft) does not appear to be. Unless they are A4 size, I wouldn't describe them as documents. And, in this digital age, they may not even exist as hard copies anyway.
