extending to the ear


Explanation:
"Gegen" and "hin" suggests movement and direction towards a certain point but that's not always the case. Many German speakers, less than elegantly, use similar phrases to describe vicinity, not movement. And that sentence doesn't seem to be worded unambiguously enough to determine that the area of discoloration is expanding.
Admittedly, you can also use spreading with the same meaning as extending, but then I would expect to be told from where to where as in "discoloration spreading form chin to ear".
