This refers to possible limitations of data gathered on foster children in a clinical setting. My guess is that the term means something about the unfamiliar nature of the surroundings and the circumstance of being a research subject, but I cannot find this term anywhere.

Thanks to all. I have now learned that several translations are possible, depending on the context. I think "rating or "assessment by someone else" is the best for my context, which involves combining several of the proposed answers--yet I have to choose only one! What it really means here is rating by the researcher, who is not all that familiar with the individual child. In other situations it can refer to grading by a teacher, or to "external" rating by an institution that is not affiliated with the one in question. But that is not at issue in my case. Special thanks to seehand who clued me in that it was a typo!

My guess is that the paragraph refers to clinicians underestimating the severity of the trauma because they overlook some symptoms present in the lower reaches of the mind that are not easily observable from the outside.
