Explanation:
Well, I'm not sure but if we are talking about the intestine than outlet might mean gastric outlet מוצא הקיבה. Other than that it just a matter of phrasing, you can use your suggestion which is completed valid in my opinion, you can my suggestion which assumes a typo and that the meaning is מרוחק and not מורחק. You can also use the hernia is distant from the [gastric] outlet, and so on. Or if it not a typo than distanced instead of distance.

Also you might consider using the word distal to the describe the מרוחק part. Without more context (that you don't have) it is only an educated guess.

Explanation:
See in the link, there is a figure showing what a hiatal hernia is (actually distant from the intestines at Esophagus base).

I believe you have to find out exactly which type of hernia it is, since there are other types of hernia, e.g. Inguinal (genital) hernia. Basically, I would assume that "source/origin remote hernia" can cover any hernia that is distant from source/origin.

It seems to me that your instinct about: 'hernia distanced from the outlet' was correct, since "outlet" is more of "a way out" or "a means of relief" than an origin.
