usually this expression indicates boredom, annoyance, sort of being "fed up" (worse) rather than anger like WTF.

Marco, could be but in that case she'd probably have said something like "... pure le palle/i coglioni t'ho dato!" Tacked on as it is (and I think the lack of punctuation only heightens the frustration of the outburst) I think it's most likely just a colourful exclamation. For those who don't live among Italians: it would be accompanied by one of a variety of "ffs" gestures that we permanent residents have learnt all too well!

It's very hard to figure out with certainty – what exactly the lady meant by those two testicles. That's because the whole phrase sorely lacks punctuation. So, bereft of punctuation, the cadence (of the balls, as it were) may mean anything from "I gave you everything; and what do I get back? F... all" to, as Marco rather creatively suggests, "I gave you everything including some damn gumption that you so severely lacked". Personally, I'm inclined towards the third way. It's more conventional, at least in Rome and the Central Italy in general, where "due/du' coglioni", or even more often "due/du' palle", or "e che palle!" simply means "what a f...ing drag! / nuisance". So, she might mean, basicaly, "I gave you everything, and what the f...ing drag it all was !"

I think the wife is throwing back at her husband's face all the things that she gave him during their life together. The two balls are among her contributions. In other words, the gentleman had no balls, she had to supply them.
