Italian term or phrase: Scheda cartacea
"Scheda cartacea per la prescrizione della specialità medicinale XXX"

This is the heading of a form (issued by AIFA) to be filled out by a physician to prescribe a certain drug. The term "scheda cartacea" also appears later in the text.
I know what the term means; my doubt is whether we would include the word "cartacea" in the translation, since it seems rather obvious to whoever is filling it out that it is a form on paper.

Or would the word need to be included to distinguish it from any electronic counterpart? Anyone had any experience with this term?

Hi Fiona, I myself would include it for several reasons. First, clients like to see a connection between the source and the target translation. Then, today, much is done on electronic media, even prescriptions so this is clearly a hard copy. Third, and I'll stop here, regardless of our desire to simplify and try to create syntactical EN structures that sound "native-like" sometimes we need to let go and get on with it without too much ado. I have one of these in my med records for a treatment plan that was prescribed to me in hospital a couple of months ago. Mine doesn't say 'scheda cartacio' anywhere, but that is what it is, since to receive the drugs I was prescribed it had to be presented to my PCGP and then to the pharmacist, even though my GP received it online and it could also have been sent to the pharmacy that way too. This treatment plan required, according to my treating physician, a paper form or I wouldn't have been able to get the prescription filled. PS you don't need to give me any points for this.
