Hpm: alta potenza di microonda del moltiplicatore (e anche qui lascerei l'acronimo; vedi secondo link)
Se fai una ricerca su Google.it con "fotomoltiplicatore OP" troverai questa abbreviazione.

Outpatient payment by visa


Explanation:
Just for the records:
I found exactly the same terms in a hospital statement issued in NY. It refers to the payment made by OutPatient using Visa card. The only term I am not sure about is Hpm that may refer to many things from Healthcare Provider Management, Hospital Policy, etc.
