  -> Thank you, Lakshmi. Re your suggestion, I think 所属先 is different from 社名 in this case since it is already asking the company name at the beginning.

Although Miho Ohashi san is in the right area the structure of the original Japanese text leads me to believe that it should be translated as "affiliation"

disagree  Takashi Koga: Normally (at least in the US), when we see "Affiliation" on a form, we think about a larger entity (e.g. association) that you belong to (E.g. Affiliation: American Translator's Association). Perhaps this may be specific to UK English?
