術前エコー診断でリッチを認めていたが、一回目(一本目)のステント留置でリッチにかかわらず2本目でステント留置を終了した。

This is describing a procedure to implant a stent in a heart (patent ductus arteriosus stenting).

立地 is the only thing I can think of here, and I suppose it makes sense as the placement site, but I just wanted to check that there isn't any other possibility I'm missing.

Yes, it is. But based on Internet search, lipid-rich is the most likely. I should have had written lipid-rich but it could initiate another argument. Asker: Do you think 'rich', an adjective, would be used as shorthand for lipid-rich plaque? Also PDA stenting is a pediatric procedure so there wouldn't be any plaque formation.
