An ideal bone marrow core biopsy sample for optimal evaluation of anemia should be approximately 1-2 cm in length. A larger core is desirable for evaluation of lymphoma or metastatic carcinoma. The core biopsy should be red, with grossly identifiable bone particles. What you do not want to see is a sample that is uniformly white or shiny. A solid white core biopsy usually indicates that cortical bone was sampled. A glistening sample may indicate that cartilage was sampled. However, metastatic carcinoma may present with a similar, glistening appearance.

JAK PRZEBIEGA TREPANOBIOPSJA?

W przypadku dorosłych pacjentów trepanobiopsję wykonuje się na jednej z kości biodrowych. Zabieg przeprowadza się w znieczuleniu miejscowym. Lekarz podaje znieczulenie w miejscu wkłucia i odkaża skórę. Następnie nakłuwa kość biodrową (kolec biodrowy) specjalną igłą do trepanobiopsji na głębokość około 3-4 cm. Kiedy igła dotrze do jamy szpikowej, lekarz usuwa specjalną prowadnicę umiejscowioną we wnętrzu igły i pobiera szpik kostny, absorbując go do strzykawki. W dalszej kolejności lekarz wykonuje odpowiedni ruch igłą, tak by usunąć ją razem z walcem kostnym, czyli fragmentem kości biodrowej zawierającym komórki krwiotwórcze szpiku. Igła zostaje wyjęta z ciała pacjenta, a w miejscu wkłucia nakłada się jałowy opatrunek.
