The text below is pretty much self explanatory, this was a transformer which was dropped and damaged during handling in the factory. I believe the accident happened while the active part was being removed/replaced, not quite sure what this term refers to, - the filling of the insulating oil reservoir, or something to do with the transformer casing itself (the tank?)??
