attending physician's report Explanation: I believe "attending physician's report" is a very commonly used way of saying this. I have provided a couple of examples below. If you use Google to look this up, you will find countless forms provided by insurers, all of them called "Attending Physician's Report," in PDF form for customers to download. Travel For accidental death or dismemberment (for all insurance plans) 1. The beneficiary or relative must notify the Company or AIAS immediately. 2.
