I think you're on the right track, but translating it is going to be difficult because we don't have a noun other than 'involvement' to refer to an affected area.

If my theory is correct, then the following restructuring might work:

1. 'The skin is often affected as a result of inoculation ...'

2. '... when two or more of a patient's organs are affected' (to avoid repeating 'involvement')

3. 'The patient with disseminated disease has involvement not only of the lungs but also the bones, urinary tract, and thyroid, as well as bacteremia. (Awkward, I admit!)
