I can understand the basic language here, but what is the intention of this line at the bottom of this hospital discharge report? Does it mean that if a copy is given to the doctor within 72 hours it is still valid?

A copy is considered for a medical PROOF/certificate - otherwise you lose your sick pay.
Explanation:
In many countries of the EU, if you leave athe hospital, you shall present yourself at your family doctor and handover him/her your hospital discharge report. If you will not do this, you lose your sick pay.

If you have transportation to come home, these costs are reimbursed from the state budget under same conditions: Train ticket second class or bus ticket.
If you were unable to go home on your own (= you are transported from the hospital, you still obliged to give a call.

Explanation:
