"Fee" is the amount paid usually to an agent/agency supplying employees to the employer. I am not that sure about the first two words "Code name" here, sorry. My guess is that you can as well translate it as "Код сделки, шифр договора, код типа договоров, название графы...." It could be just the name of a standard line in this or that type of agreement.
