Great research as usual, Charles, your entry looks correct to me, only I won't give an "agree" as I don't feel qualified to do so.
Just from a linguistic standpoint, it might also be possible to simply leave it as "cauda" or "tail".

Here are a few relevant entries from Stedman's Medical Dictionary:

cauda, pl. caudae (kaw′dă, kaw′dē)

...

And also, regarding "caudate", I assume from the following entry that the term means "taillike" or "having a tail".
