Scheme You can use scheme, plan or program. Scheme is fine in my opinion, for we lack context and despite the fact although it sometimes has negative connotation like for example in the “field” of financial fraud (i.e. Ponzi Scheme). Since this is a bank statement, one presumes, there is no chance of fraud.

Not much more info from context Since this is a bank statement, everything is just a line item, and there is not much more context...could "insurance plan" also work? "Esquema" is a tricky word when translating it into English.
