declared before a notary / has made a notarized declaration to that effect

Explanation:
A couple of ideas.

"...in a stable de facto relationship declared before a notary..."
"...in a stable de facto relationship, who has made a notarized declaration to that effect..."

(confirmed) by a notarized declaration

Explanation:
This de facto union was confirmed by way of a declaration, probably written, that was notarized by a notary public.

El union de hecho fue confirmado o verificado por medio de una declaración, probablemente escrito, que fue notarizado, es decir, la firma y la identidad de la persona fue verificado por un notario y registrado con él.

disagree  Angelo Berbotto: I am afraid I disagree here. La unión (no el unión) is not confirmed by the notary. The parties make a declaration and this declaration is solemnised in the deed issued by the notary.

-> Yes, I should have written "la unión" but de facto unions may exist without being declared (see for ex. https://www.notaria19bogota.com/la-union-marital-hecho/). Notarizing gives legal proof of something that pre-exists the declaration.
