We reach the heart with minimal incision near the arm pit and using a guidewire that goes from a large vein (near the arm pit) all the way up to the heart - or in this case, all the way up to the device implanted in this patient's chest (and all the equipment removed or inserted is done so through this guidewire). Considering that the Spanish and English syntax are very different I believe it's easier to start in the middle of the sentence and work your way to the beginning.
