Asker: Thanks Cecilia, that's what I currently have put in my translation, though I didn't have "full" which I think makes it clearer, so thank you for that. My doubt is whether, to be even clearer, it should say "assumes full responsibility should they disregard...." to avoid it reading as if they are actually responsible for disregarding the medical team's recommendations (which presumably would be an undesirable thing to do).

Yes, of course I understand that it is the patient who may choose to disregard the medical team's recommendation, but clearly this would not be what the medical team would want. And there is an ambiguity (for me at least) in saying the patient "assumes responsibility for disregarding" because it could read that it is one of their responsibilities to actually disregard, whereas in fact what it means is: if you disregard our medical team's advice, it's on your own head (not ours).... I realise I am tying this up in knots somewhat, but I want to get it absolutely right and clear ;-)

agree  Giovanni Rengifo: I like this option with "disregard", and Communican's right about changing the wording from "for disregarding..." to "....should the patient disregard." Also, "full" is not in the source, but it makes it sound legal. I like that!
