I found this for the interesting reference to Garp (not, as it were, 'The World According to ...)

Asker: I think you are on the right track. To answer your question, this document was quite sloppily written generally, but notably it did not use a bunch of abbreviations or shorthand so I did not immediately think of this as being a short form.
