Source text - English This unit provides an overview of normal hepatic structure, including: • The size and location of the liver. • The hepatic blood supply and route of blood flow. • The regenerative capacity of the liver, which is particularly significant. • When the organ is damaged or diseased. The liver is the largest gland in the body and the largest organ after the skin. The average weight of the adult liver is about 1.4 kg.1 The liver is located in the upper right part of the abdominal cavity, with the upper border lying approximately at the level of the nipples as indicated in figure 1.2 Patients with chronic hepatitis B may experience mild abdominal pain in this area or in the mid‐abdominal area.3 The upper and anterior surfaces of the liver are curved to fit under the diaphragm. The posterior surface is irregular due to the adjacent abdominal viscera.4 The liver is anatomically divided into 2 major lobes which are separated by the falciform ligament — and termed the left and right lobes. The large right lobe is divided into 2 lesser lobes: • Caudate lobe (on the posterior surface of the liver) • Quadrate lobe (on the inferior surface of the liver)2 Figure 2 illustrates the lobes of the liver and the falciform ligament. Various structures enter and leave the liver through the portal fissure, which is a region on the posterior surface of the liver. These include the hepatic portal vein and the hepatic artery, both of which supply the liver with blood.4 The hepatic portal vein, hepatic artery, and other structures in the portal fissure are described in the table. Several structures that are adjacent to the posterior surface of the liver are also important to consider. These include the inferior vena cava, which is the vessel that carries blood away from the liver.4 In addition, the gallbladder, a pear‐shaped sac about 7 cm to 10 cm long, is located in a depression on the posterior surface of the liver.1 Bile produced and secreted by the liver is sent via the hepatic ducts to the gallbladder for storage, to be released as needed.1 Lobules Liver tissue is organized into lobules, which are columnar groups of cells comprising structural units. Each hexagonal lobule is 0.8 mm to 2 mm in diameter and several millimetres in length. Each lobule is arrayed around a central vein, which is a tributary of the hepatic vein.
